aging-disabilityThe VT-ADRC envisions every Vermont community, inclusive of seniors, all persons with disabilities, Veterans, the Deaf, and family and chosen caregivers, having easy access to a full range of coordinated programs, services, information and supports throughout Vermont.  The ADRC will illuminate consumer access to an integrated and well-funded statewide system of information and assistance, options counseling, care transitions, and streamlined access to long term services and supports across the lifespan.

The Vermont Aging and Disability Resource Connection (VT-ADRC) is a collaboration among trusted, consumer-centered, non-profit organizations. The VT-ADRC provides comprehensive and objective information and options counseling to Vermonters seeking services, benefits or information related to long-term care services and supports for aging, disability and independent living. By contacting an ADRC partner agency, Vermonters have the ability to make informed decisions to live with independence and dignity in their chosen communities for as long as possible, with the types of supports and services they choose.
